Each of the 3 measured outcomes (fidelity, reach, and sustainability) will have its own set of questions, with fidelity being the most finely tuned, with an initial probing questionnaire that will eventually develop into a mature and finalized version.
Fidelity is a crucial outcome that we will closely monitor, as it ensures consistent program implementation across different settings.

To fully understand why interventions are ineffective or effective we need to understand whether these interventions are designed, delivered, and received as planned (fidelity). Without an assessment of fidelity, the reported effectiveness of an intervention could be a function of either an effective intervention or the influence of other unknown factors added to, or omitted from, the intervention [19].

This study explored the fidelity of the national rollout of Healthy Living to the original He LP-Diabetes intervention. Intervention fidelity is defined as the extent to which an intervention is delivered as intended [16]. Without good fidelity to the original He LP-Diabetes intervention, there would be no strong justification for the implementation of Healthy Living, and reasons for intervention effectiveness would be unclear.

An Alternative to Traditional Bedside Teaching During COVID-19: High-Fidelity Simulation-Based Study
Simulators can be classified according to the degree of realism known as fidelity. High-fidelity simulators provide the user with immersive and often complex scenarios with a high degree of realism [9]. Low-fidelity simulators replicate the real world to a lesser extent and include part-task trainers, for example, an inanimate model of a limb to allow learners to practice venipuncture skills [9].

While evaluations of intervention effectiveness have not yet been performed, in the meantime, we sought, as planned, to examine the initial fidelity of the intervention’s ability to engage KT recipients beyond the point of care through the use of monthly invitations to complete brief portal assessments that allowed them to report on their regimen adherence and any specific concerns.

Intervention fidelity is used to describe whether an intervention is delivered as intended [15,16]. Without an assessment of fidelity, it cannot be ascertained whether intervention effectiveness, or a lack thereof, is because of the intervention content or other factors added or omitted in the intervention delivery.

The gold standard methods for training require considerable provider and expert trainer time for didactics, skills practice, fidelity or quality assessment, and feedback or coaching sessions [22,32,33]. Therefore, continuing education generally requires taking large amounts of time away from work at considerable financial costs to the trainee and their employer.

Interrater reliability for fidelity coding was excellent (96.5% concordance). Fidelity to the intervention was high for all grades, though some seventh- and tenth-grade classrooms were not able to proceed through all 5 scenarios due to timing constraints. These instances were noted distinctly from fidelity because they resulted from longer-than-expected time completing the pretest survey immediately prior to the intervention, a factor that would not exist outside of the study.
