We will approach hospitalized, English-speaking, African American patients at UCMC, aged 18-70 years, who meet the criteria for advanced CKD (Stage 3b or greater) with an estimated glomerular filtration rate (e GFR) This study will be a substudy of the Hospitalist Project, an ongoing clinical study that examines a variety of outcomes of patients hospitalized in our general internal medicine services, and includes the analysis of administrative data, inpatient interviews, and 30-day follow-up by phone [7].
