Mention of CBT-I, benzodiazepines, trazodone, and other antidepressants increased over time, while nonprescription terms showed varying fluctuations. Our results both align with prior studies and present new findings, particularly for treatments that are not well captured in medical records.

In line with these findings, clinical research has confirmed that objective knowledge about antidepressants is positively associated with patients’ beliefs about their value and willingness to use them [45,46]. Approximately one-third of patients discontinue antidepressants within the first month of treatment and 44% discontinue them by the third month of treatment [6]. Kales et al [7] examined veterans recently prescribed antidepressants and found 29% nonadherence based on self-report and 53% nonadherence when based on the medication possession ratio using veterans Affairs (VA) pharmacy data.

Even so, more than one-third of patients discontinue a new prescription within 3 months [2], and similar rates of discontinuation are also seen when increasing dosage, combining antidepressants, or changing antidepressants [3]. Nonadherence increases the risk of deterioration of psychiatric symptoms, with an increased number of emergency visits and hospitalizations, resulting in not only great suffering at an individual level but also an increased economic burden on the health care system [4,5].

Antidepressants are the first-line treatment for depression that is considered as “more severe” (or moderate to severe depression), for which they have been shown to be effective [3-5]. Current guidelines advise that antidepressant treatment should continue for 6-12 months after remission of symptoms; however, research indicates that there has been an increase in long-term use (≥12 months) [6].

Early 5-HT alterations, either via pharmacological, genetic, or other manipulations, potentially alter these processes via the presynaptic, membrane-bound serotonin transporter protein (5-HTT), the target of SRI antidepressants. 5-HTT is a key regulator of brain 5-HT [26,27].

Understanding Side Effects of Antidepressants: Large-scale Longitudinal Study on Social Media Data
Importantly, antidepressants are known to show varying effects across individuals and conditions [5]. Despite several meta-analyses [6], existing evaluations of the benefits and harms of antidepressants are based on group data and can only serve as advisories for individual patients. Due to clinical heterogeneity, an individual’s subjective experience is crucially important to consider but is difficult to be incorporated.

Additionally, depressive symptoms and disturbances in self-perceived cognition followed different trajectories during treatment with antidepressants; improvements in cognitive function were more gradual than improvements in mood symptoms, with a lesser magnitude [8].
